To be fair, a large part of why OSU was made an example of was because it took the NCAA snooping around before they actually did anything. It wasn't like out of nowhere OSU came out and said, "Hey, we had these minor problems and just found out about them, but just wanted to let you guys know."
That said, the NCAA sucks at enforcing punishments in any type of consistent basis. The whole "Failure to Monitor" reason is a joke and should just be done away with.
